Form A is to be used when applying for a British passport for a person aged 16 or over.

• Application forms are available from the passport office, the main post office and most parish halls.
• Submit two passport sized photographs, one with a countersignature on the reverse.
• The fee of £72.00 is payable in cash, by cheque to the Treasurer of the States and credit or debit cards.
• For renewal, please provide your previous passport.
• Further instructions can be found in the explanatory notes enclosed with the form. You may be asked for other information to ascertain your nationality when submitting the application.

Form B is to be used when applying for a British passport for a person under 16 years of age.

• Application forms are available from the passport office, the main post office and most parish halls.
• Include a full birth certificate if it is the first application and there is no other passport to hand in.
• Mother's passport.
• Any other passport the child appears on.
• Submit two passport sized photographs, one with a countersignature on the reverse
• The fee of £46.00 is payable in cash, by cheque to the Treasurer of the States and credit or debit cards.
• Further instructions can be found in the explanatory notes enclosed with the form. You may be asked for other information to ascertain your nationality when submitting the application.

The Jersey Passport Office issues passports to British citizens in Jersey and to Jersey born British citizens who are in the United Kingdom.
