Are you likely to qualify for the scheme?

1

You are 65 or over at the time of application
and

YES 

2

you do not pay income tax because your income level is below the relevant tax threshold
and

YES

3

you have been resident in Jersey for 5 years
and

YES

4

if you are single your capital assets are less than £20,000
or if you are married less than £30,000

YES

If you answer yes to all of these statements you are likely to be eligible to register under the Jersey 65+ Health Plan.


Part One - What is the Scheme?

The Social Security Department is operating a Scheme, funded by the States of Jersey, to subsidise dental, optical and chiropody costs for the over 65’s.

Claims for financial assistance may be made up to the maximum sum allowed in any benefit period. The benefit period begins from your date of registration with the Scheme. The date of registration is confirmed in the welcome letter you will receive from Westfield. The benefit period is one year for dental and chiropody subsidies, two years for optical and two years for those claiming subsidies for dentures.

Please note: you will not be able to make a claim for treatment you have had before your registration has been confirmed.

Financial assistance from the administrators will be available as follows:

Optical

  • Every two years, up to £15 towards an eye test
  • Every two years, up to £90 towards new prescription spectacles, lenses or contact lenses

    Sundry items such as contact lens solutions, frames only, non prescription spectacles and repairs are NOT covered by the Jersey 65+ Health Plan.

    Dental

  • Every year, up to £22 towards a dental check-up
  • Every year, up to £250 towards dental treatments or dentures

    Please note: members claiming for dentures (either a full lower or upper set OR a complete set) can claim up to double the maximum subsidy, once in a two year benefit period. If you have already received some dental treatment and made a claim during the period, this amount will be deducted from your remaining subsidy allowance. For example, if you have received a dental subsidy of £50 you would only receive £450 towards your dentures.

    Sundry items such as toothbrushes, dental floss etc. are NOT covered by the Jersey 65+ Health Plan.

    Chiropody

  • Every year, up to £90 towards the cost of Chiropody treatment from a State Registered Chiropodist (a list of State Registered Chiropodists can be found in the advertising section of the Jersey Telephone Directory).

    Part Two - Who is Westfield ?

    Westfield Health has been appointed to underwrite and administer the plan on behalf of the States of Jersey.

    Founded in 1919, they are a nonprofit making organisation dedicated to providing value for money healthcare plans for the benefit of their contributors.

    Customer Service

    With their excellent customer service record, claims are usually paid within 2-5 working days following receipt of the claim form. Payments can also be made direct to your bank account.


    Part Three - How do you know if you qualify?

    The criteria to qualify for the Scheme are listed below. Please read this list to see if you qualify.

  • You are 65 years of age at the time of application.
  • You do not pay income tax because your income level is below the tax threshold. (Please telephone 445505 if you require further information).
  • You are currently resident in Jersey and at some time you have been resident in the Island for a continuous period of five years.
  • If you live alone your capital assets are less than £20,000.
  • If you are a couple (married or cohabitating) your capital assets are less than £30,000.

    N.B. Capital assets do not include your own home but do include items such as other owned properties, savings and investments, pension bonds and other items of value such as jewellery and paintings.

    Everyone eligible to qualify for the Scheme must complete and sign an Application Form which you will find at the back of this leaflet. You should then return it to the Social Security Department as soon as possible.

    If at any time your circumstances change so that you are no longer eligible for the Scheme, you must inform the Department.

    Part Four - What happens when you have completed the application form?

    When you have completed the application form please send it to the Social Security Department. The details, as submitted on your form, will be reviewed and if verified will be passed to Westfield. They will administer your application and process your enrolment on to the Scheme, on behalf of the Social Security Department.

    Westfield will confirm your membership by sending you a welcome letter with further detailed information about how the Scheme will work in practice e.g. how to make a claim.

    Please note: you must have received confirmation from Westfield that you have been accepted into the Scheme before you submit a claim.

    N.B. The Department will ensure that all information provided to Westfield will be held in accordance with the current Data Protection legislation.


    Part Five - Appointing an Agent

    If you are unable to sign the claim form and wish to appoint someone to act on your behalf, you can complete a Form of Authority. Ask the relevant people to complete this form and, if you are a new applicant, return it to the Social Security Department with your application for membership. If you are already a member, you should send the form directly to Westfield.

    A Form of Authority can be found at the end of this leaflet. If for some reason the form is missing, please contact us on 445505.


    Part Six - How to make a claim

    Visit the Dentist/Optician/State Registered Chiropodist

  • Receive treatment
  • Pay for your treatment
  • Obtain a receipt (Please note, credit card receipts are not acceptable)
  • Forward this receipt plus the completed claim form to Westfield

    You can have your subsidy paid directly into your bank account. A Direct Debit form is included in your Welcome Pack from Westfield. If you would rather have money paid into your bank account than receive a cheque, complete this form and send it directly to Westfield.

    Westfield will process your claim in accordance with the rules of the Scheme. They will then either send you a cheque or advise you that they have applied a credit directly to your bank account.

    Further details will be available in the Welcome Pack which is sent to you when your application is verified.

    Part Eight - Other help you may get

    Income Support 

    Income Support is an income-relate benefit that provides finanacial support towards the costs of housing, living, medical needs and childcare.  Carers also receive support.

    Income Support is available to individuals and families that satisfy condiyions relating to residency in Jersey and employment.

    The amount of Income Support payable depends on income, savings and assets, and on personal circumstances.
