Work alongside Islanders living with any disability or long-term condition, and includes support for:
The team help Islanders overcome everyday challenges and get the support they need, when they need it. This includes access to repurposed care and mobility equipment through their Equipment Hub.

Patient and visitor advice and support (PALS): Health and Care Jersey
Provide confidential advice, support and information on health-related services or care. They can provide a point of contact for patients, their families and their carers by:
- answering any questions you have about Health and Care Jersey
- listening to your concerns about your care
- putting you in contact with the right services, teams or organisations
- listening to your suggestions on how we can improve our services and direct you to the feedback team

St John Ambulance
Free Carers Support Programme, including guest speakers, to support family members and friends who care for someone in the community.
St John Ambulance: Carers Group
Monthly meet-ups for unpaid carers, offering guest speakers and social activities.
Following the Carers Support Programme the group provides:

St John Ambulance: Companion Scheme
Weekly companionship and respite. Trained, DBS-checked volunteers are carefully matched with individuals who:

A fortnightly social group run with Call and Check. Open to independent, mobile adults, with:

Jersey Hospice Care
Provide free, holistic palliative care for anyone in Jersey with a life-limiting condition. They offer support:
- at their In-Patient Unit
- in the community at home
- through bereavement services
They focus on improving quality of life by managing symptoms and providing emotional, spiritual, and practical support for patients and their families.

Telecare
Telecare helps you live safely and independently at home by providing emergency support and monitoring if you:
- are vulnerable
- are elderly
- have physical or learning disabilities
- have long-term conditions or complex needs
The Telecare service uses:
- alarm pendants. These are worn on your body and allows you to speak directly to a Care Navigator
- sensors, including:
- fall detection
- smart plugs
- seizure monitors
- environmental devices
- GPS location tracking, which is used to find someone
- Braille on buttons and other aids, if you’re visually or hearing impaired

Macmillan Cancer Support Jersey
Provide free, confidential emotional, practical, and wellbeing support for anyone affected by cancer in Jersey through their Oasis centre, by phone or online. Support is provided for patients, as well as their family and friends.

Jersey Youth Service: My Time 4 Young Carers
Provide support for young people from age 8 to 18 years, who care for someone with a support need. This could be due to:

Jersey Community Foundation
Provide grant funding support to charities, community and voluntary groups in Jersey whose work benefits our community.
Examples of available funding include:
- The Greville Bathe Fund. This was set up to support sick and elderly Islanders
- The Ann Alice Rayner Fund. This was created to help people living in Jersey who have financial difficulties
