Digital consent
Digital consent, also called eConsent, is a way to give consent for treatment. Instead of signing a paper form, you review and sign electronically using a system called Concentric.
It gives you more time to think about your decision and talk it through with family or friends before signing.
Digital consent helps you:
- understand your treatment options more clearly
- receive information from your clinician in a simple and secure way
- make decisions together with your clinician, at a time and place that works for you
- reduce the need for paper forms, supporting a greener health service
The consent information can also be printed to take home.
Compatible devices
You can access your digital consent form on any touchscreen device with internet access such as:
- mobiles
- tablets
- laptops
- desktop computers
If you don’t have access to a touchscreen device or can’t sign digitally, it won’t affect your care.
You can sign in person when you next see your clinician or on the day of treatment.
How it works
Before your procedure, your clinician will explain the treatment and create a digital consent form tailored to your care.
You can receive your digital consent request by:
When you receive your digital consent request, you’ll need to enter the date of birth you’ve shared with Health and Care Jersey to open your consent form securely.
You can then:
- read it at any time, at home or in hospital
- read it as often as you like
- take time to think about the information and ask questions
Signing the form
When you’re ready, use your finger or a compatible electronic pencil to sign your digital consent form on screen.
If you’re a parent, guardian or have legal responsibility for someone else, you can sign on their behalf. Your clinician will guide you through the process.
Once completed, a copy of your digital consent form is automatically saved in your Health and Care Jersey patient record.
On the day of your treatment, you confirm your consent verbally.
Your clinician records this in Concentric. You don’t need to bring your consent form as your clinician already has access.
Checking it’s secure
When you receive your digital consent request, check that:
- text messages show Concentric as the sender
- emails come via Concentric using
notifications@concentric.health
- the subject line is: Important information about your upcoming treatment
- the message starts with Dear, and your name
- it includes the Health and Care Jersey logo
- the link in the message is unique to you and can only be opened when you enter your date of birth

Data privacy
Concentric uses Google Cloud Platform (GCP), hosted in a UK data centre. GCP meets all healthcare information governance standards.
Your data is protected using strong encryption and access controls. You receive a unique link and enter your date of birth to verify your identity. All data is stored and transferred securely.
