About the Jersey Care Model​

Jersey is on an exciting journey towards implementing a new model of care including the build of a new hospital that will transform health and social care provision in Jersey

The overarching aim of the Jersey Care Model (JCM) is to transform health and social care, in order to improve Islanders' physical and mental health and wellbeing. To achieve this, it adopts a person-centred approach whereby care is financially sustainable, safe and accessible, being provided in the places where people need it the most.

About the roles

The ambitious five year improvement programme is led by the Improvement and Innovation Directorate within the Health and Community Services Department. We are now looking for passionate people across a range of professions to join our multidisciplinary team to contribute to the design and implementation of the Jersey Care Model. This is an exciting opportunity to join an expanding team and to contribute to the sustainable health and wellbeing of Islanders.

Funding has been agreed for a five year programme team to support the delivery of the JCM. All advertised roles:

  • are fixed term until the end of 2025, unless otherwise stated
  • require entitled status or five years' residency in Jersey, unless otherwise stated

There are a variety of opportunities at all levels across the five teams of the Directorate. 

Strategic Planning and Reporting (including Health and Care Policy)

The strategic planning and reporting team drives and supports the department to exercise good business management and supports the department with policy and strategy development. The team champions a robust corporate governance culture and ensures ministerial, political and audit requests are being coordinated and completed at good quality and on time by working closely with the Care Groups, the Ministerial Support Unit and other government departments.

Portfolio Change Management Office (PCMO)  

The PCMO team provides the holistic overview of all change activities, it provides project management support for improvement projects including the Jersey Care Model (JCM) programme and the secretariat for the JCM governance framework.

Quality Improvement 

The QI team drives continuous improvement and learning across the department will embed a QI approach in clinical and operational areas. The team will work closely on improvement projects and with the PCMO team whilst also developing a dedicated QI training programme. One focus area for the team in 2021 and 2022 is the support for the Mental Health improvement plan and to support operations with implementing processes for continuously improving the waiting lists.

Intelligent Client Lead for Digital Health

The role will be a strong intelligent client for digital health and develop a productive supplier/client relationship with the central IT function (Modernisation & Digital) who delivers digital health on behalf of the Health and Community Services department.  The role acts as a subject matter expert to lead on the strategic change plan for digital systems and digital integration elements, providing the interface between the CCIO, clinical and operational health and care service leads, the Improvement and Innovation Directorate and the Modernisation and Digital team.
