Mental health support after CAMHS
We support young people aged 17 and a half to 25 who are moving from
Children and Adolescent Mental Health Services (CAMHS) to adult mental health services.
If you’re finding this change hard, we’re here to help make it clearer and less stressful.
We work with you if you have ongoing mental health needs and want support, stability and continuity as you move into adulthood.
Our main aims are to:
- help you move smoothly from CAMHS to adult mental health services
- understand your needs early and plan your transition with you
- make sure your care continues without gaps
- support your wellbeing, confidence and independence
Using the service is your choice and we’ll work with you for as long as you need us.
We keep you involved and informed, so you feel confident and in control of your care.
How to access the service
Referrals must come from CAMHS. If you feel you need support and move to adult services, speak to them first. They can offer advice and refer you if needed.
You may be referred if you:
- are approaching the upper age limit for CAMHS
- have ongoing mental health needs and need continued support
- would benefit from planned support when moving into adult services
Once we accept your referral, we work closely with CAMHS, you and your family or someone important to you to support your move into adult services.
What we help with
Our support is tailored to your needs to make sure your move from CAMHS to adult services is smooth and well-supported.
We can help with:
- an early assessment to understand your mental health, and your social, education and development needs
- planning your move from CAMHS to adult mental health services, working with you and CAMHS
- one‑to‑one support to help you build confidence, coping skills and independence
- clear information about mental health, medication and managing your wellbeing
- practical support with education, work, housing and everyday life
- help to access adult mental health services, including community support, talking therapies or specialist care
- involving your family or someone important to you, where this is helpful
- signposting to charities, youth groups and local community support
About our team
Our Transitions Mental Health Team includes:
- a clinical psychologist
- a psychiatrist
- mental health nurses
- a support worker
We work closely with other services and organisations to get you the right support at the right time.
