Getting support in the community
We provide assessment, treatment and ongoing support in the community for adults with severe or long-term mental health needs.
This includes conditions like:
- schizophrenia
- psychotic disorders
- bipolar disorder
- severe depression
- severe anxiety disorders
- significant mental health problems
Our aim is to help you recover, stay independent, improve your quality of life and avoid hospital stays by supporting your wellbeing in the community.
How to access the service
To access this service, speak to your GP or another health professional who can refer you.
If you’re already under the care of a mental health team, your care coordinator can also make a referral.
Once you’re referred, your information is reviewed and sent to the Community Mental Health Team. They’ll contact you directly to make an appointment and discuss next steps.
You can choose to pay for private therapy with a counsellor, psychologist or psychiatrist. You’ll need to find a clinic yourself and pay for all costs. Find how more about

What we help with
We can help you:
- understand your needs and plan the right support
- manage distress, build coping skills and work towards recovery
- get support with your medicines, including advice and regular reviews
- get help with mood, anxiety and psychosis, and prevent relapse
- stay safe by giving you compassionate, non‑judgemental support
- look after your physical and mental health
- get help with housing, benefits, relationships and daily living
- find or stay in work, training or volunteering
- access local health, social care and voluntary services
Your main contact will be your care coordinator. They will help you make a care plan, help organise appointments, update professionals about your care and make sure you get the right support.
When you’re feeling better, we’ll plan your next steps and arrange ongoing support with your GP or our other services.
