Getting support in your home
We support you at home if you’re an adult in crisis or need short‑term help to avoid going into hospital.
We work with you if you’re very unwell but can be safely supported at home:
- instead of going into a mental health ward in hospital
- after you’ve left a mental health ward in hospital
We aim to help you recover in familiar surroundings, close to your family, friends and daily routine.
Our aim is to:
- support your recovery at home
- help you through a crisis
- support an early discharge from hospital
- help you stay well and avoid relapse
- work with your family, carers and other services
We focus on keeping you safe and supported while helping you rebuild confidence and independence.
How to access the service
If you’re receiving care by a mental health professional and they feel that home support would help you, they’ll make a referral.
The Home Treatment Team will review your information, decide what support you need and contact you to discuss next steps.
What we help with

- visiting you every day
- helping you manage symptoms
- supporting your medication
- supporting you during difficult or high‑stress times
- offering practical and emotional support for you and your family
- working with your community mental health team to plan next steps
- supporting an early discharge from hospital and helping you settle back at home
We personalise your care by adapting our visits, support and treatment to meet your needs.

The Home Treatment Team includes:
- mental health nurses
- psychiatrists
- occupational therapists
- support workers
- social workers
We also work closely with other teams to make sure your care moves smoothly between services.
