​Getting assessed for memory loss and dementia

Our team assesses, diagnoses and supports adults who:

  • have memory problems or changes in thinking
  • are worried about memory loss, confusion or difficulty concentrating

We help you understand what may be causing your symptoms, access the right treatment or support, and live well with your condition.

We also help you and your family understand your diagnosis.

If you have memory difficulties, we support you to stay independent, understand your condition and maintain your quality of life.

How to access the service

To access this service, speak to your GP who can refer you.

They will:

  • talk with you about your symptoms
  • carry out initial checks
  • refer you if appropriate

Once you’re referred, your information is reviewed and sent to the Memory Assessment Service if you need a specialist assessment. They’ll contact you directly to make an appointment and discuss next steps.

Waiting times for assessments vary. Our aim is to see you within 13 weeks, but you may have to wait several months.

What we help with

We can help you with:

  • initial assessments, including medical history, memory tests and talking through your symptoms
  • specialist diagnosis by clinicians with experience in dementia and related conditions
  • medication support, where appropriate, to help manage symptoms
  • support after diagnosis, including advice and signposting to local services
  • support for families and carers, with information on the condition and how to help

We work closely with GPs, social care and voluntary organisations to make sure you get the right care at the right time.

About our team

The Memory Assessment Service team includes:

  • a specialist doctor
  • specialist mental health nurses
  • a consultant psychologist
  • an occupational therapist
  • a support worker

We work together to provide compassionate, person‑centred care for you and your family.
