Memory checks for people with learning disabilities (easy read)

If you or someone who knows you well are worried about your memory, you can talk to your doctor, nurse or social worker.

The Adult Learning Disability Team are here to help you.

We can ask for you to have a memory screening assessment.

An assessment means you will be asked some questions to see how you remember, think and do things.

We can also ask for you to have a health check to see if any other health conditions are affecting your memory.
