​It can tell people about your health and medication, how you communicate and any support that you need. 

​If you need to go to the doctor or to the hospital you can take your hospital passport with you to show people. 

You can ask a family member, your carer or support staff to help you fill it in. 

You can keep it and add or change information when you need to.
