​How many individuals have been referred from the General Hospital to a care home in the last 18 months?

Response

The number of individuals discharged from Jersey General Hospital to a Nursing Home or Residential Care Home bed each month, for the 18 months preceding receipt of this request, are detailed in the attached table. Figures represent unique patients discharged in each month, and include those discharged back to a care home as their usual place of residence.

It is not possible to report upon open referrals or referrals to a care home which were not accepted or otherwise unsuccessful from the Patient Administration System.

In addition to the above, records show 141 discharges to Jersey Hospice Care over the same period, calculated from unique patient discharges each month.​
