Find out if you have a learning disability (easy read)

We work in the Adult Learning Disability Team.

We want to find out if we can help you.

We can meet with you to see if you have a learning disability.

This is called screening.

You will need to have an assessment.

An assessment means we will ask you some questions so we can find out more about you.

We can see you at our offices at our Government building on Union Street in St Helier or we can visit you in your home.

Your screening appointment

It is helpful to have someone with you who can help answer these questions.

You can bring a family member or one of your support staff who has known you for a long time.

We can ask to speak with someone on the telephone if they cannot come to the appointment with you.

At your appointment we will ask you some questions about your past.

We will ask about your childhood and when you went to school.

We will ask about things you find easy or difficult to do now.

We will ask you about your life and how you feel.

We will ask you about things you enjoy.

The assessment takes around 1 hour.

We might need to see you again on a different day to ask more questions.

Your screening report

We will be making notes, this helps us remember things you said.

The notes will be written up into a report.

Only other people in the Adult Learning Disability Team will be able to see the report.

This is so they can help you too.

If there is anything you don’t want people to know, tell us and we can talk about it.

Once the report is ready, we will write you a letter.

What happens next

The letter will say if you have a learning disability or not.

We will then ask to see you.

We will talk to you about what will happen next.

If you do have a learning disability we can help you.

If you do not have a learning disability another team might be able to help you.

If you feel worried or have any questions about your screening appointment, contact us.

We look forward to meeting you.
