Individual support

The Children and Families Hub team can offer individual support to make positive changes for your family. This can take place in your home or we can meet you somewhere else that suits you (for example in a café, or outdoors).

You can also call or email the Children and Families Hub to discuss your family’s needs on +44 (0) 01534 519000 or email childrenandfamilieshub@gov.je.

Early help

Early Help gives children and families the support they need, at the right time.

A practitioner will talk with your whole family to understand your needs. They will listen to any challenges that you or your family are experiencing and summarise the discussion in an Early Help Assessment. This will consider the needs of your whole family and help everyone understand your family’s situation.

If you think you and your family might benefit from some support, you can ask a practitioner who you know. This could be your doctor, health visitor or a teacher at your child’s school. They can tell you more about Early Help.

Sometimes a practitioner may approach you and ask whether you would like to talk about Early Help and how it could help your family.

To contact the Children and Families Hub call +44 (0) 1534 519000 or email childrenandfamilieshub@gov.je to ask about Early Help for your family.

Short Breaks

If your child has a disability and is unable to attend mainstream activities, the Children and Families Hub co-ordinates targeted short break opportunities for children aged from 4 to 18. These include weekend, after school and school holiday opportunities provided by Centrepoint Trust and Youth Service.

You can find out more information about Short Breaks for Children with Disabilities or you can contact the Children and Families Hub on +44 (0) 1534 519000 or email childrenandfamilieshub@gov.je.

Play and Connect drop-in sessions

We offer a Play and Connect drop in session every month at the Bridge Child and Family Centre. The sessions promote families’ wellbeing by providing a welcoming space for parents and carers to play and connect with their children.

Practitioners from the Children and Families Hub, the CAMHS Early Intervention Team and Mind Jersey are available to support play and provide an opportunity for parents and carers to discuss any concerns or worries they may have about their child’s development or mental health.
