I am writing to request information under the Freedom of Information (Jersey) Law 2011 regarding the administration of vaccinations at the Fort Regent vaccination centre.
Please provide responses to the following regarding vaccinations received:

Where are the records stored for individuals who received Covid-19 vaccinations at Fort Regent?
Vaccination Types Administered:
Please confirm which types of vaccines were administered at Fort Regent between 2020 - 2024, including:
Covid-19 vaccines (brand names such as Pfizer-BioNTech, Moderna, AstraZeneca, etc.)
Flu vaccines, including the brand names used (e.g. Flucelvax, Fluenz Tetra, etc.)
Access to Individual Vaccination Records:
Are members of the public request able to access details of any vaccines given at Fort Regent between 2020 - 2024:
Where can members of the public obtain a copy of the consent they gave to receive a Covid-19 or flu vaccine at Fort Regent?
Where can members of the public access the batch number(s) of the vaccine(s) they were administered?
Where can they obtain the exact dates of administration?

Vaccines administered at Fort Regent between 2020 and 29 December 2023, when the vaccination centre closed, are detailed in the attached document:

Vaccine certificates were issued to individuals who completed both their first and second Covid-19 vaccinations. Digital certificates were also available, upon request.
Records for individuals who received vaccinations at Fort Regent vaccination centre are stored in the COVID-19 EMIS instance. Members of the public can request access to their vaccination records through a Subject Access Request.
Batch numbers of vaccines administered and dates of vaccination are held in the COVID-19 EMIS records. Confirmation of consent obtained is also documented. Adults who attended Fort Regent for vaccination were not required to sign a consent form; verbal consent was sufficient. Where a consent form has been signed, including in the case of parental consent for children and Best Interest forms, a scanned copy is retained in the EMIS record.
