A report outlining the progress made in the first 12 months of a five-year programme to redesign Jersey's health and care service has been published.
The Jersey Care Model Update Report 2021 highlights the work that has been undertaken so far.
The JCM, which was approved by the States Assembly in November 2020, aims to keep Islanders living healthy and independent lives for as long as possible and, if they do require health and care services, that these are centered around the patient and, where clinically appropriate, delivered in the community.
The Jersey Care Model is a five-year programme of work which has been split into three delivery tranches. All targets set for 2021 (tranche 1) have been met and work scheduled for 2022 has already commenced. These deliverables include:
- Recruiting and developing a team of clinical and project staff to lead and support the delivery of the programme;
- Establishing good governance to ensure the JCM delivers on its strategic intentions;
- Developing new intermediate care services, such as Overnight Community Care, Help at Home and Telecare, to help reduce unnecessary hospital or residential care admissions;
- Setting up a formal commissioning structure with health and care partners across the Island, to create a more co-ordinated collaboration of health and care support for Islanders;
- Investing in Public Health in order to understand Jersey's specific health and care needs which will enable the prioritisation of interventions to help improve health and prevent ill health across the Island population;
- Developing an island-wide workforce strategy to support the on-going training, recruitment and retention of health and care professionals across the Island;
- Progressing the delivery of a new electronic patient record system.
During the second phase of the JCM, which runs during 2022 and 2023, Islanders will begin to see a more integrated system making access to health and care easier and more consistent.
Deputy Richard Renouf, the Minister for Health and Social Services, said: "The Jersey Care Model is an ambitious programme aimed at giving Islanders the health and care services that they deserve. I'm proud to say that in its first 12 months it has made sound progress in laying the solid foundations required to making the Model successful.
"I hope that this report inspires confidence in the JCM and the benefits it will have for our community and would like to thank all those that have been involved in delivering the programme so far."
