Lifeline Care Group is expanding and currently seeking two dedicated Care Coordinators to join our growing team. These roles are key to maintaining the highest standard of person-centred care across our services. You will be responsible for developing care plans, managing staff rotas, conducting risk assessments, and ensuring smooth delivery of support to clients in the community. Candidates must have excellent communication, leadership, and organisational skills, with a flexible, hands-on approach. You’ll work closely with clients, families, and external professionals to uphold quality care, while supporting and supervising frontline care staff. A Level 3 qualification in Adult Care (RQF) or equivalent is required, along with relevant practical experience. Full induction and ongoing training provided.
