We are launching a Sustainable Primary Care Strategy for Jersey.
It is the first time that such a strategy has been published.
It proposes that services which provide primary care for Islanders in Jersey become more efficient and work closer together in the future.
Representatives from the primary care sector and Social Security have worked closely with Health and Social Services to bring it to fruition over two years.
Primary care services in Jersey (eg GPs, dentists, optometrists, pharmacists and community nursing) are those services which Islanders use on a regular basis for their healthcare needs.
They are services which Islanders will use for the whole of their lives.
As Health and Social Services re-design health and social care services for Islanders to cater for the needs of a growing elderly population and to allow people to continue to live in their own homes as long as possible, one of the areas which is being looked at in detail is the way in which all healthcare services in Jersey link together; and this includes primary care.
HSSD have already been able to work with primary care colleagues in order to put investment into primary care services, including:
- Jersey Talking Therapies
- health visiting
- development of rapid response teams
- the Community Midwifery initiative
- reablement services
- end of life care
- alcohol services
The Sustainable Primary Care Strategy sets out five ambitions which have come about resulting from the consultation, which relate to:
- patients
- payment
- partnerships
- people
- processes
