Community based mental health services for people over 65

​This service is for people aged over 65 who have mental health problems. The community mental health service consists of:
  • community mental health team 
  • memory clinic
  • day assessment unit
  • day care services

Some people may need care and services in a hospital-based residential setting. Clinique Pinel and Rosewood House provide services in these cases.

Accessing community mental health services

The community service and memory clinic only take referrals from general practitioners (GPs). The community mental health team meets every week to decide if a referral is appropriate for the memory clinic or the community mental health team, and this meeting also provides an opportunity for the team to discuss individual cases.

What we do

The community mental health team works to provide care for people in the most appropriate environment for each individual. Some of the things we do include:

  • providing information and support to clients and carers
  • monitoring conditions
  • organising respite
  • managing medication
  • assessing care placement, risk management, daily living activities, homes and adaptations
  • psychological therapy

Who we work with

We work with a number of agencies, such as:

  • GPs
  • Family Nursing and Home Care
  • private care agencies
  • Housing Department
  • Social Security Department
  • charities
  • parishes
  • respite and continuing care team
  • day care services
  • other Health and Social Services departments, including the hospital itself

The memory clinic

The memory clinic accepts referrals from GPs, mainly for people over 65 who are experiencing difficulties with their memory. The memory clinic offers support with:

  • assessment
  • diagnosis
  • treatment
  • information
  • monitoring

Where appropriate, memory clinic staff will refer people to other agencies, including the community mental health team.

The memory clinic is available by appointment during office hours.   ​
